Megumi has two choices while dealing with Tsumiki: battle her to save others, or pick her over his friends and the rest of the colony. Tsumiki’s body may have been possessed by a powerful old sorcerer, albeit this is only a rumour at this point. Megumi knows Tsumiki isn’t a sorcerer since she saw no indications of it before the Culling Game. Even more disturbingly, he doubts her identity and physical control. And thus, in Chapter 211 of “Jujutsu Kaisen,” Megumi is forced to take action.
In the end, he may have to fight Tsumiki, but that might have negative consequences, especially if he has to murder her.
